SmartFTP will not recognise my host

When adding the information to the new browser form and clicking OK, the authorisation fails. I have entered my correct username and password for my site and I double checked with the host to check I had it correct. Any help appreciated.

If you are unable to login there are 2 reasons for that:
1. host/username/password not entered correctly (most likely case)
2. your account is disabled on the server
